Options Community Services is a non-profit registered charity providing social services primarily in Surrey, Delta, White

community.
The Health and Wellness Consultant at Options Community Services will be responsible for developing and
implementing programs and recreational activities to support the health and wellness of clients. The role will involve
working closely with clients to support the development of individualized recreation and wellness plans, as well as
collaborating with other members of the Options Community Services team to coordinate and deliver high-quality
health and wellness services.
ACCOUNTABILITIES
- Develop and implement programs and recreational activities that support the health and wellness goals of
- Work collaboratively with clients and staff to develop individualized recreation and wellness plans that align
- Monitor and track health and wellness program effectiveness, collect and analyze data, and adjust programs as
- Coordinates care for clients and implements therapeutic interventions. Develops, implements and adjusts
multidisciplinary team, consults with client’s general practitioner and/or community agencies and involves
family where possible.
- Monitors and documents client’s mental status, response to medications and progress. Assists client with
variety of health issues.
- Collaborates, consults and provides clinical support to colleagues for continuity of care, treatment concerns,
based upon specialized theory/practice, discusses client care issues and developing linkages/partnerships with
other facilities, agencies and support groups.
- Coordinating and facilitating group events such as educational seminars, events and meetings related to

QUALIFICATION
Education, Training and Experience
- Master's degree in health sciences, kinesiology, recreation therapy, or a related field.
- Minimum of two (2) years of experience in designing and implementing health and wellness programs and
- Must be eligible for certification with a professional association.
Knowledge, Skills and Abilities
- Knowledge of health and wellness principles and practices, including nutrition, exercise, stress management,
- Understanding of the social determinants of health and their impact on health outcomes.
- Excellent written and verbal communication skills.
- Knowledge of behavior change theories and techniques.
- Familiarity with relevant laws and regulations related to health and wellness programming.
- Ability to build and maintain positive relationships with clients and community partners.
- Experience working with individuals with diverse needs and backgrounds.
- Strong interpersonal and communication skills, including the ability to build rapport with clients and
- Excellent project management skills, including the ability to develop and implement programs and initiatives
- Ability to work independently and take initiative, as well as collaborate effectively with a team.
- Ability to identify and leverage community resources to support health and wellness programming.
- Ability to adapt to changing priorities and handle multiple tasks and projects simultaneously.
Special Requirements:
- Personnel must consent to and pass a criminal record check as per agency policy.
- Personnel must sign a Confidentiality Agreement and Code of Ethics.
- Must complete a six (6) month probationary period before staff are permanent employees, unless otherwise
