You will be responsible to ensure that the geriatric specific psychosocial needs of residents followed by the Geriatric Consult Service are identified. You will provide support to residents followed by the Geriatric Consult Service and their caregivers, in collaboration with other health care providers in the circle of care. You will communicate and collaborate with teams on issues related to discharge planning, and coordinate referrals to other geriatric services. Your contributions to the organization’s Quality Improvement Initiatives related to Geriatrics are essential.

Responsibilities

  • Clinical MSW and/or BSW from an accredited university. MSW is preferred
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Membership with the Ontario Association of Social Workers an asset
  • 3-5 years’ clinical practice experience with a geriatric patient population
  • Effective interpersonal skills combined with the ability to work in collaboration with all members of the healthcare team, both internally and externally, across the continuum of care
  • Flexibility and adaptability to change is essential
  • Demonstrated ability to facilitate change and continuous improvement processes
  • Excellent organizational, planning and time management skills
  • Reliable individual with excellent attendance record
  • Excellent knowledge of MS Office, RAI-MDS
  • Communicate and collaborate with other healthcare providers for residents who are followed by the Geriatric Consult Service
  • Provide recommendations regarding an approach to psychosocial needs of our older adult residents
  • Promotes quality residents outcomes by facilitating effective processes of care and utilization of resources
  • Proactively identifies and addresses geriatric-specific barriers to client transitions across the continuum of care
  • Leads engagement of residents, families, members of the interprofessional team, leadership and community partners in identifying strategies and action plan for the management of frail seniors
  • Applies and models health literacy principles (i.e. Teach-Back)
  • Applies and models behaviour-based coaching methods
  • Maintains an in-depth knowledge of internal and external resources and services to support client transitions to appropriate settings for care,, such as mental health services, Regional Geriatric Program, etc.
  • Maintains an in-depth knowledge of Long Term Care (LTC), Home and Community Care Support Services (HCCSS), Behaviour Supports Ontario and Alternate Level of Care legislation and processes
  • Facilitate, lead and participate in appropriate client transition processes
  • Enhance interdepartmental, inter-hospital and interdisciplinary collaboration, communication and problem solving
  • Develop primary and secondary specialization to support transitions with populations of interest and health systems

HOURS OF WORK:
Monday - Friday, days, week ends

Schedule:

  • 5-8-hour shift
  • Hours varies (Day Shifts)
  • Has the desire to work with our vulnerable population and provide exceptional service
  • Driver's license, and reliable transportation is a must
