POSITION SUMMARY

Under the direction of the Health Programs Manager the LPN will be responsible for providing routine care, observing patients’ health, assisting doctors and registered nurses, and communicating with patients and their families. This will also include improving the quality of life of the client (community members) by assisting them to make healthier lifestyle choices; promote client dignity, independence, comfort, mobility, personal appearance/hygiene, and safety. The LPN may also need to provide support to the Community Health Nurse (CHN) and /or the Home and Community Care (HCC) nurse.

WHAT YOU’LL DO

  • Advise community members of available health services and programs through NIHB and medical services.
  • Perform nursing functions using established procedures, policies, guidelines, and standards.
  • Contribute to the development of the patient/significant other plan of care and discharge plans based on the identified patient problems/needs.
  • Document pertinent observations and interventions on appropriate records.
  • Conduct the initial home visit and refer clients to HCC and/or appropriate services.
  • Conduct short-term individual, family, and group counseling in the field of health.
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record.
  • Primary Care including dressing changing, monitoring vital signs etc.
  • Report client needs/problems and observations regarding their condition to the registered nurse and physician.
  • Administer the patient travel services according to the patient travel policies and procedures.
  • Maintain and distribute clinical supplies.
  • If needed, assist in coordinating accommodation for community members traveling for health reasons.
  • Perform foot care procedures to clients, as required.
  • Identify gaps in services and inform the Manager and the Department Team.
  • Provide Home Support services including meal preparation, personal care, palliative care, transportation, and transfer of functions as requested by the HCC/CHN nurse.
  • Provide follow-up visits to monitor the health of clients including pre-natal, hospital discharge and other illnesses as requested by HCC/CHN nurse.
  • Maintain professional and accurate client service records.
  • Prepare all necessary documentation for community members as needed.
  • Assist with live birth certificates and monthly reports to vital statistics.
  • Attend scheduled team meetings with the Health and Social Development Department staff members, and other meeting as requested by the Health Services Program Manager.
  • Be familiar and updated with Health Canada policies and procedures governing the delivery of community health services.
  • Sign and maintain strict oath of confidentiality.
  • Demonstrate adherence to department Safe Practices Standards.
  • Other duties or responsibilities as assigned by your supervisor, Director, or the CAO.

WHAT YOU BRING

Education

  • Completion of Licensed Practical Nursing Program from accredited institution required.
  • Registration and licensing with BCCNM.
  • CPR level “C.”

Experience

  • Three years’ experience working in a First Nation community or rural setting.
  • Must be adept for fast-paced environment with a variety of responsibilities.
  • Ability to balance competing priorities and to work under pressure.
  • Flexibility and creativity.
  • High professional ethical standards.
  • Proficient computer skills including Microsoft Office Applications (Word, Excel, Internet, and Email).
  • Excellent active listening, verbal, and written communication skills.
  • Ability to adhere to Oath of Confidentiality and Seabird Island Band Policies and Procedures.
  • Knowledge, respect and understanding of Stó: lō culture, traditions and language is considered an asset.
  • Valid BC Driver’s License, Class 5.
  • Satisfactory Criminal Record Check.
