In a Primary Care Network (PCN), a group of family doctors, together with Alberta Health Services coordinate health services for patients. The Highland PCN includes the communities of Airdrie, Beiseker, Carstairs, Crossfield, Didsbury and surrounding areas. The Highland PCN encompasses 23 clinics, 69 physician members and serves over 70,190 patients. Several health care professionals work closely with physicians to support patient care.

As a member of the PCN’s Health Team, the clinical Social Worker is responsible for assessment, therapeutic intervention, and consultation services to support youth aged 7-17 with anxiety, depression and/or ADD/ADHD. This position includes the provision of patient and family therapy, assessment, treatment, care coordination, education, consultation and collaboration for youth and their families. This role works in partnership with other health professionals, including family physicians. A strong understanding of child and youth mental health programs, community parenting resources, the ability to refer to and liaise with these services as well as experience providing mental health assessments and therapeutic interventions are required.

Accountabilities and Responsibilities
Assessment, Intervention, Education and Advocacy

  • Provides short-term patient and family-centred counseling and intervention to children and youth in accordance with evidence-based therapeutic treatment modalities
  • Assists patients and their families/support network, achieve their optimal physical, emotional, mental, spiritual health and well-being
  • Collaborates with and provides consultation and information to parents to develop and support the goals and treatment plan
  • Detects and addresses a broad spectrum of behavioural health needs with the aim of early identification, quick resolution, long-term prevention, and wellness
  • Facilitates routine screening, risk assessment, and intervention (including mandatory reporting where applicable) for risks including suicide/self-harm, abuse, domestic violence, neglect, and child welfare concerns
  • Teaches group psychotherapy classes
  • Coordinates services and care with the medical home team and others involved in the circle of care to ensure the patient’s needs are met in a timely manner
  • Provides family physician with same day feedback on patient encounters

Team Responsibilities

  • Collaborates with other health care and community service providers to ensure a continuum of mental health in the community
  • Promotes local programs and services that support patient care
  • Ensures the maintenance of up to date and accurate records in the Electronic Medical Record (EMR) to optimize patient care and adhere to the practice policy
  • Participates in data collection and evaluation as required
  • Offers training and consultation to the PCN health team, and physicians on complex cases and where psychosocial issues impact patient care
  • Initiates and maintains positive community partnerships

Qualifications and Experience

  • Minimum of a bachelor’s degree in Social Work, MSW preferred
  • Current unrestricted registration with ACSW
  • Authorization to perform restricted psychosocial interventions required
  • 5 or more years of experience, preferably in a primary care setting
  • Experience working with children, youth and families experiencing mental health concerns
  • Family/individual therapy and/or behaviour change training or experience
  • Addictions and/or behavior change training or experience is an asset
  • Experience in rural health preferred
  • Experience with Electronic Medical Records (EMRs)
  • Ability to work days, evenings, and weekends
  • Valid driver’s license and own vehicle
  • Fully vaccinated against Covid-19

  • Leadership, problem solving, conflict resolution and critical thinking skills
  • Excellent written and verbal communication skills
  • Experience in working in a collaborative, consultative manner within an interprofessional team
  • Knowledge of medications
  • Maintains up-to-date resources of patient education materials and resources for commonly seen problems
  • Sound judgement, self-directed, flexible, and displays initiative in a new and evolving role
