Responsibilities of Registered Practical Nurse (RPN):

· Acts as a resource and provides leadership to other health care providers collaborating in and overseeing care provided by unregulated health care providers. This includes: assigning tasks within their scope of practice and providing guidance and support as needed.

· Participates in auditing all aspects of resident care to ensure quality care is delivered to each and every resident by planning, initiating, directing and evaluating resident care.

· Liaises with outside agencies and organizations including other LTC facilities, MOH, medical, diagnostic and pharmacy services.

· Participates in related nursing functions such as but not limited to infection control, restorative care, social work, health records, nursing library, staff development, skin care, oral and dental care, and ADL's.

· Maintains clear and accurate nursing Focus documentation in accordance with Documentation Standards. The Registered Practical Nurse ensures that Transfer of Accountability is achieved and maintained at all transfer points.

· Refers problems requiring medical attention to the RN promptly for assessment and follow-up with the resident's physician for treatment.

· Consults with Registered Nurse in interpreting clinical information from sources such as physician orders, patient charts and patient/client histories and collaborates with the RN when the care needs of the patients are outside his/her scope of practice.

· Responsible for giving medication as ordered by the physician.

· Responsible for ensuring that the documentation of resident care is accurate and concise.

· Uses nursing processes to assist in identifying resident needs and developing care plans and to keep care plan updated and current at all times.

· Ensures that workplace incident/accident forms are completed promptly and reported to the RN.

Requirements of Registered Practical Nurse (RPN):

· Current registration in good standing as Registered Practical Nurse with the College of Nurses of Ontario

· Medication Administration Course

· Current CPR certification

· Basic Foot Care Course an asset

· Experience in a Long Term Care setting preferred

· Courses in Gerontology, Palliative Care, skin care and wound management, Alzheimer's, dementia care and psycho-geriatrics are an asset

· Must have the ability to apply knowledge, skill, judgment and competencies required for safe independent and interdisciplinary practice by meeting nursing standards as set out by the CNO and related professional associations. I.e. medication administration.

· Must possess sufficient computer skills. Experience with PointClickCare preferred.

· Must be able to demonstrate excellent communication skills - verbal, non-verbal, and written.

· Must have the ability to complete reports, orders or other documentation clearly and accurately

· Must adhere to relevant Provincial and or Federal regulations as well as Conmed/CPL policies and procedures including the collection use and disclosure of personal health information.

· Must possess organizational and planning skills and the ability to work independently sufficient to complete multiple tasks concurrently.

· Must act as a patient/client advocate and encourages others to be receptive to their needs.

· Must have the ability to assist residents, family members with concern and empathy; respecting their confidentiality and privacy and communicating in a courteous and respectful manner.

· Must have the ability to take appropriate action to ensure safety of self, patient and families, volunteers and other team members by adhering to Conmed/MTM policies and procedures.

· Must possess English language verbal communications skills and literacy skills sufficient to understand verbal and written instructions.
