Scope of work and responsibilities:

• Identification and assessment of home therapy suitability.
• Education of patient in the home therapy chosen i.e. home haemodialysis or home peritoneal dialysis.
• Assessment of patient during monthly clinic visits.
• Liaison with community and industry partners to ensure that supplies and technical requirements of dialysis are met.
• Collection and documentation of dialysis quality data.
• Willing to travel to the patient’s home to conduct home visits.
• To participate in the development of educational materials congruent with adult learning.

Qualifications include:

• Current registration with the College of Nurses of Ontario
• Current BLS Certificate.
• Nephrology Nursing Certificate required. Enrolled in Nephrology Nursing Certificate program upon hire with successful completion required within 3 years may be substituted if there are no qualified applicants.
• A minimum of 3 years experience in dialysis required. Home dialysis experience preferred.
• Demonstrated ability to design and implement patient education programs.
• Superior oral and written communication skills
• Competency in computer word processing and spreadsheets
• Excellent time management skills with flexible attitude to working hours
• Knowledge of Baxter dialysis equipment an advantage
• Good attendance record
