Job Summary

The Professional Practice, Patient Experience Lead and Clinical Informatics Specialist conducts meaningful analyses that generates data-driven insights and recommendations so clinicians and leaders can make better decisions to improve patient outcomes, patient experience, provider experience, and reduce cost/increase efficiency. This position develop and implement programs to ensure the provision of safe, quality patient care and assist in creating inter professional practice environment that fosters creativity, growth, professional judgment, and high practice standards.

Education

Baccalaureate degree in Nursing required.

Master’s degree in Nursing from a recognized university preferred.

Conflict management training or Alternative Dispute Resolution certification an asset..

Course program in adult learning principles/educator certification or equivalent required

LEAN training certification preferred.

Experience

Minimum 5 years' experience in an acute care setting.

Minimum 3-5 years clinical educator/teaching experience.

Experience and demonstrated clinical proficiency required.

Minimum two years of hospital experience working with a major Health Information System e.g. MEDITECH, is preferred.

Experience with clinical and educational program development, implementation and evaluation.

Demonstrated knowledge of relevant legislation and legal frameworks including Patients First Act, Public Hospitals Act, Excellent Care For All Act, Health Quality Ontario’s Quality Improvement Plan, Freedom of Information and Personal Privacy Act, Personal Health Information Protection Act, Personal Information Protection and Electronic Documents Act, Quality of Care Information Protection Act, Ontario Human Rights Code, Accessibility for Ontarians with Disabilities Act, Occupational Health & Safety Act, Corporate operational policies and procedure.

RESPONSIBILITIES

Patient Experience/Relations

Support staff on the use of conflict management and alternative dispute resolution skills enabling all levels of staff to identify, intervene, and mediate patient satisfaction /service recovery in a timely fashion.

Coordination and implementation of an integrated system to manage patient/family feedback.

Collection, collate, analyze report data on a quarterly basis to evaluate trends and facilitate quality improvement and satisfaction in patient care.

Support program and corporate change projects and continuous improvement initiatives based on employee, physician and patient satisfaction data.

Professional Practice

Plans, implements and evaluates learning opportunities for nursing staff utilizing relevant research and best practice evidence.

Promotes an environment that facilitates clinical questioning and learning.

Maintains learning attendance records and monitors the implementation of educational activities, making revisions as necessary to achieve goals/objectives.

Excellent understanding of adult learning principles.

Proficiency in critical appraisal of research literature and application to practice

Maintain current knowledge of, and monitor, emerging professional practice issues, legislation and standards and advises and make recommendation to direct manager and leadership team.

Participates actively in quality improvement initiatives to facilitate changes in nursing practice and care delivery models.

Facilitate meetings with clinical stakeholders in preparation for the implementation of programs/equipment.

Clinical Informatics

Participate in customizing, testing and implementation of applications, upgrades and enhancements to the Meditch application and other health information systems.

Complete training on vendor products being implemented.

Document current state workflow and identify challenges that require mitigation during the transition to future state.

Develop and implement processes relating to professional practice throughout the hospital which support the implementation of interdisciplinary evidence-based patient centered care delivery systems.
