Reporting to the Manager of Seniors’ Services, Palliative Care & Primary Care, the Occupational Therapist (OT) will provide care to patients in the Seniors’ Services program, which includes, but not limited to, supporting the emergency department and outreach. The OT works collaboratively with other health professionals, frail older adult patients, families / caregivers and community partners to: enable the best possible health, maximize function and support frail older adults to live safely at home as long as possible. The Occupational Therapist will demonstrate a high level of autonomy and skill in developing patient-centred occupational therapy services in keeping with the College guidelines and standards of practice for patients with occupational performance needs. The Occupational Therapist provides services to patients referred to Seniors’ Services program that serves the older adult population throughout the Mississauga Halton region. In addition, the Occupational Therapist may be involved in other clinical, research, evaluation, education, or leadership activities, as needed.

Principle Responsibilities Include:

  • Provides specialized geriatric, assessment, care planning treatment and follow up on a consultative basis; for frail older adult patients in their homes. Particular attention is paid to assessment of cognitive function and the impact this may have on the older adult’s ability to perform their day-to-day tasks and to inform a diagnosis.
  • Provides short term interventions in the home environment and/or referral to and consultation with relevant providers or agencies.
  • Provides education, support and resources for frail older adults and their caregivers.
  • Provides consultation and assessment for mobility devices. May also assist with providing resources and application for equipment funding as needed.
  • Develops effective partnerships with physicians and members of the inter-professional team to provide a high quality of care to patients and their families.
  • Participates in program development, education and community liaison for frail older adults within the region.
  • Participates as a member on relevant internal and external committees and professional groups.
  • Participates in Occupational Therapy professional practice activities.
  • Involved in ongoing quality improvement initiatives in the delivery of service.
  • Contributes to the overall vision/mandate of Seniors’ Services.

Key Qualifications:

  • Registered to practise with the College of Occupational Therapists of Ontario.
  • Candidate will have a minimum of 3 years of relevant experience including experience in homecare and with the geriatric patient population. Experience in geriatric medicine an asset.
  • Has a keen interest in managing the care of frail older adults with a range of diagnoses.
  • Demonstrates the ability to work with a dynamic, inter-professional team which includes families and community partners.
  • Able to perform cognitive assessments using a variety of assessment strategies / tools.
  • ADP authorizer for mobility devices.
  • Demonstrates effective interpersonal and communication skills.
  • Demonstrates excellent organizational and time management skills.
  • Sound knowledge of community partners and programs is an asset.
  • Has a good understanding of evidence-based Occupational Therapy practice and how it interfaces with those of other health care disciplines.
  • Must have a valid Ontario driver’s licence, relevant insurance and use of own vehicle, and must travel across the Mississauga Halton Local Health Integration Network.
