MAIN DUTIES:

Reporting to the Manager of Continuous Quality Improvement, the Nurse Practitioner (NP) is a primary care provider to residents and works within his/her legislative scope of practice as described and outlined by the College of Nurses of Ontario. The Nurse Practitioner collaborates with the resident and family/care giver, along with our interdisciplinary care team in the development, implementation and evaluation of our residents’ plan of care; provides leadership and mentorship to other team members; and leads and collaborates research, education, and evidence-based practice initiatives.

QUALIFICATIONS:

  • Proof of current registration with the College of Nurses as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice. Masters Degree in Nursing preferred.
  • Post-graduate education and experience in gerontology is an asset.
  • Specialty certification in gerontological nursing is an asset.
  • Post-graduate certification and experience in palliative care is an asset.
  • Knowledgeable about primary health issues in long-term care facilities.
  • Education and / or experience working with responsive behaviours is an asset.
  • Education and / or experience working with people living with dementia is an asset.
  • Excellent written and oral communication skills.
  • Ability to work with other health care professionals.
  • Valid driver’s license and a reliable vehicle with insurance is required.
