Nurse Practitioner Recruitment:

Located in northern Perth and Huron Counties in Southwestern Ontario (Listowel, Wingham and Area), the North Perth-North Huron Family Health Team is a progressive team of health professionals with an integrated approach to healthcare. We are currently seeking a Nurse Practitioner, 0.8 FTE (30 hours per week). Position Summary:
The Nurse Practitioner will work predominately in Wingham with the possibility of some travel to our Listowel, Teeswater and Lucknow clinics. The NP will work in close collaboration with physicians and allied health care professionals within the Family Health Team (FHT) and community partners. As a vital member of the patient focused team, nurse practitioners will provide primary care services in conjunction with all health care professionals. Nurse practitioners will focus on acute/episodic care, health promotion, disease prevention and chronic disease management in a primary care setting with a goal to assist patients and their families to function optimally within their capacity. The initial focus of nurse practitioners will be to provide increased access to acute/episodic primary care services for residents of North Perth and North Huron (Listowel, Wingham and surrounding area). Nurse practitioners will also assist in the ongoing development of primary care programs, resources in collaboration with other members of the FHT and other partners. The scope of practice of the nurse practitioners will follow the guidelines published by the College of Nurses of Ontario.

Qualifications:
Qualifications for this position will include:

  • Current registration with the College of Nurses in Ontario in the Extended Class
  • The ability to work in a rapidly changing environment
  • Experience with program planning and evaluation
  • Willingness to work in a collaborative practice setting with other allied health professionals
  • Have previous experience in working with patients and groups of patients
  • Have demonstrated ability to work independently
  • Possess well developed organizational, communication, teaching and leadership skills

Job Type: Full-time

Salary: $52.00-$58.75 per hour
