Delivery Driver/Community Service Technician – Transitional Care

Full Time

Monday-Friday 9:00am-5:00pm

The Toronto Grace Health Centre (TGHC) provides, specialized care to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Transitional care and Palliative Care services. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church& Bloor St), Harbour Lights (Queen & Jarvis Street) houses and a Specialized Care Centre (Victoria Park & Mc Nicoll). POSITION SUMMARY

Reporting to the Manager - Transitional Care & Assess and Restore at the Salvation Army Toronto Grace Health Centre is currently seeking a Delivery Driver/Community Service Technician . The Transitional Care program provides safety devices (e.g. falls-alert/SOS pendant, Karie medication dispensing device, etc.) to eligible clients in the Greater Toronto Area. Safety alerts are monitored and responded to by the Transitional Care 24/7 call centre, and devices are installed by the team’s community service technicians. The successful candidate for this position is responsible for establishing and maintaining excellent relationships with clients by providing courteous, efficient and professional delivery/installation service in a safe and timely manner. The Delivery Driver/Community Service Technician performs functions relating to delivery and exchange of Karie cartridges and medications to clients’ homes, as well as facilitating installations of the program’s safety devices. Providing in-home technical support for clients is also required. This position will report to the Manager of Transitional Care & Assess and Restore and is a great opportunity to become a part of a growing and innovative program. .

Qualifications:

· High School Diploma completed or recognized equivalent

· Must have access to a car and have a valid Class G Ontario Driver’s license in good standing

· Applicants must be at least 21 years old

· 1 year of experience delivering to and from medical clinics or pharmacies, or similar technical role preferred

· Ability to learn how to use/configure new equipment and troubleshoot, as needed.

· Experience with older adults and other marginalized populations is an asset

· Comfortable visiting clients’ homes

· Strong computer & software/technical skills

· Excellent English language speaking, reading and writing skills

· Ability to work independently with minimal supervision

· Exceptional customer service, effectively listen, strong problem-solving skills, flexibility, efficiency and strong time-management skill

· Able to operate GPS for navigation within the GTA (e.g. Google maps, etc.)

· Comfortable driving in all weather conditions

· Ability to sit, stand, be mobile and operate a vehicle for extended periods of time

· Must have the physical strength, flexibility and endurance required to perform the role, as determined by the needs of the position and based on assessment conducted by Occupational Health Services

· Vulnerable Sector Check from your local police department will be required

· Ability to speak a second language is an asset.

Main Responsibilities:

· Operates motor vehicle to make various deliveries throughout the GTA and developing efficient routes within the Greater Toronto Area (as defined by the City of Toronto) to patient homes in a timely manner

· Working with GRTHealth (RCM’s partnering organization) to develop efficient installation and maintenance schedules.

· Prepare, inspect and test equipment, prior to installations, as required.

· Reviewing orders before and after delivery to ensure the orders are complete and the client is satisfied

· Providing exceptional customer experience, addressing client and family concerns regarding the equipment, its functions, and troubleshooting, as required.

· Accurately maintain delivery, installation logs and equipment maintenance

· Complete all documentation in a timely manner

· Demonstrate a professional image, including being professionally attired, with a name badge.

· Adhere to all Personal protective equipment requirements as per Infection Prevention and Control

· Perform other duties as required
