As a Coordination & Resource Supervisor within our CBI Health Home Health Services team, you will support a group of direct care providers and Care Coordinators to ensure that clinical service delivery meets or exceeds standards set by CBI Health and external stakeholders. Showing the way as a Regulated Health Professional (RN or RPN), you will support regulated staff (RN/RPN) in the provision of services that align with their scope of practice, with a focus on ensuring service and clinical excellence.

This role is a Full Time position, that requires you to work in office (at our Simcoe location) and an On Call Supervisor component which may require travel in the field throughout Simcoe, Haldimand, Norfolk and Brant.

Your role

  • Provide leadership and coaching to the Care Coordination Team
  • Accountable for coaching and troubleshooting with the coordination team to provide a superior and responsive patient and family experience
  • Train, onboard and mentor new coordination staff
  • Coach team to exercise due diligence in following procedures, processes, and guidelines
  • Support performance expectations with team and communicate how performance goals are linked to organizational goals and values
  • Create a team environment that fosters strong employee engagement
  • Participate in afterhours on call support for the on-call nurses and on-call coordinators
  • Support the investigation, resolution and reporting of all customer related complaints
  • Ensure accurate and timely completion of appropriate documentation, including client charting
  • Actively identify potential risks with internal stakeholders to mitigate and/or manage risk, with particular attention to missed visits and clinical care
  • Ensure a timely and comprehensive response to Incident Reporting (IR)
  • Supports Field Staff Scheduling and Coordination as part of the Coordination Team
  • Effectively resolve or escalate customer issues as appropriate
  • Manage the staffing requirements for the coordination team based on call volumes to ensure stakeholder needs are met
  • Ensures appropriate staffing levels day to day, covering sick days, staff shortages

What you will need to be successful

  • Possess a current registration with College of Nurses of Ontario (CNO) as a Registered Nurse or Registered Practical Nurse
  • Have 2 years of supervisory experience and/or experience in the home health service delivery
  • Be able to effectively establish and build rapport with clients and care staff
  • Possess a valid Ontario driver’s license with access to a reliable vehicle with ability to travel in community
  • Have the ability to participate in afterhours on call support for the on-call nurses
  • Have the ability to provide best possible solutions to problems that align with policies and uphold best practices
  • Be self-motivated with an ability to work both independently and as an effective member of a team
  • Preference will be given to those candidates with prior coordinator experience in a community health organization
