JOB SUMMARY

Purpose of the role: In keeping with the Mission Statement and to maximize and maintain occupancy, the incumbent is responsible for the coordination of all LTC and Respite application, admission, discharge, and transfer procedures. The incumbent is the direct liaison with the HNHB Home and Community Care/ Placement team, and other community services relevant to the admission of residents to St. Joseph’s Lifecare Centre.



KEY ACCOUNTABILITIES & SUCCESS CRITERIA

  • Performs all functions in accordance with the Mission Statement, Organizational Philosophy, Administrative policies, and procedures of St. Joseph’s Lifecare Centre
  • Manages the Bed Occupancy and reports monthly the occupancy rate. Alerts the Director of Long-Term Care/Administrator when the occupancy is at risk of being <97%.
  • Initiates and maintains communication and collaboration with HNHB Placement Services with regards to applications, denials, admissions, and discharges of residents. Reviews applications and responds through Health Partner Gateway (HPG) within required timeframes.
  • Provides customer service to applicants, families, care teams, hospitals, and community agencies.
  • Receives all application forms; coordinates and collects necessary information and signatures/consents (application form, power of attorney, admission agreement, etc.).
  • Coordinates application forms and maintains admission, transfer, and discharge records in keeping with facility policy and legislation.
  • Coordinates, negotiates, and processes all short-stay and permanent admissions to St. Joseph’s Lifecare Centre.
  • Statistical data collection and distribution for short stay and permanent admissions reported through CQI monthly and quarterly. Prepares quarterly reports for the CQI committee.
  • Maintains confidentiality of all records and information relevant to application, admission, transfer, and discharge, pertaining to applicants, families, and residents of St. Joseph’s Lifecare Centre, for release according to relevant policies.
  • Participates on committees, working groups and teams as appropriate, approved, or directed.
  • Serves as back up to the nursing department. Meets with resident and family. Facilitates the completion of admission documents and orientation to the RHA and the care team. Provides emotional support and guidance during the admission process.
    Assembles resident charts in coordination with the Ward Clerk.
  • Schedules post admission meetings for residents in the absence of the Ward Clerk.
  • Responsible for completing necessary tasks in the admission process/checklist.
  • Responsible for referring residents to BSO (Behavioural Supports Ontario) at the point of admission based on risk screening.
  • Participate in the annual required program evaluations
  • Participates in the development of resident’s initial plan of care with timeliness and resident specific focus
  • Continuously develop, implement, and evaluate quality improvement/best practice initiatives and participate in Continuous Quality Improvement Activities
  • Supports the nurse management team, may also be required to support and contribute to the development of clinical staff including but not limited to:
  • Coordinates and oversees the work of the RAI department
  • Identifies program gaps and works collaboratively with staff involved and other resources in the development and improvement of program plans


QUALIFICATIONS & SKILLS


  • Diploma in Practical Nursing and current registration with the College of Nurses of Ontario
  • Previous experience with LTC Admissions and existing knowledge of RAI MDS 2.0
  • Experience and aptitude using various computer programs including MS Office Suite (Word, Excel, PowerPoint) and Health Partner Galaxy
  • Clerical skills including use of Excel, access, word, Health Partner Gateway an asset.
  • Knowledge in the Fixing Long-Term Care Homes Act, Rules, and Regulations (2021)
  • Effective service skills to liaison with the public and community agencies
  • Superior communication skills. Including a demonstrated ability to articulate and communicate guidelines or policies to others clearly and accurately.
  • Excellent organizational and problem-solving skills.
  • Ability to accept responsibility and initiate appropriate procedures.
