Wages: $45.93-$59.29/hr
Reporting to the Director of Clinical Services- Community, the Nurse Practitioner (NP) will work in partnership with a multi-disciplinary team to provide safe, quality care to clients residing in the community. The Nurse Practitioner will work to the full scope of practice: ordering screening, diagnostic investigations, diagnosing and prescribing interventions as required based on current research and best practice. In addition, the NP will lead a team of nurses assuming responsibility for delegation, coordination, and delivery of healthcare services.

DUTIES

You will lead a team of health care professionals to deliver comprehensive direct and indirect care to medically complex seniors and their families, in both clinic and home settings. You will work closely with members of a multidisciplinary team, as well as with clients, families and community agencies. Working within your scope of practice, you will complete assessments, diagnoses (where appropriate), treatments, evaluations of clients and caregivers and their medical/social/psychological needs, and perform related administrative tasks. You will be involved with case management and perform referrals to appropriate team members or outside agencies as required.

Educational and other Qualifications

  • Master’s Degree in Nursing as a Nurse Practitioner with a focus on advanced nursing practice
  • Active registration with provincial professional association (CARNA)
  • Gerontology certificate
  • Designated Capacity Assessor
  • AADL Authorizer
  • CPR Certification
  • Valid Class 5 Driver’s License
  • Two years as an Advanced Practice Nurse in geriatrics with a collaborative, flexible community based approach
  • Five years health related experience and knowledge in a combination of Acute/Sub-Acute/Rehabilitative Community Care Programs and Services
  • Three years leadership/supervisory experience

Knowledge, Skills and Abilities

  • Coordination of a care team in an interdisciplinary setting; promoting positive care and a quality focused culture
  • Advanced clinical nursing skills to care for medically complex geriatric clients
  • Provides clinical supervision, education and mentoring for students and clinic nurses.
  • Demonstrates effective conceptual, organizational, interpersonal, advanced critical thinking and problem solving skills related to clinical care and services utilizing available resources
  • Works collaboratively with the interdisciplinary team and community partners to address emerging senior’s needs in the community
  • Demonstrates an in-depth understanding of the clinic program, deliverables and reports which will require knowledge and skills for data collection
  • Demonstrates flexibility by changing routine to accommodate client/clinic needs

Requirements:

  • A current and clear Criminal Records Check Form and if applicable a Vulnerable Sector Check that has been obtained no more than 90 calendar days prior to the hire date.
  • A tuberculosis screening that has been completed no more than 12 months prior to the hire date.
