Right at Home is currently seeking a Client Care Coordinator to become part of our team in Abbotsford.

The Client Care Coordinator, is responsible for all aspects of client home care, including introductions, client assessments, care planning, and quality assurance. This position will also handle service inquiries, scheduling employees and maintains scheduling information and data as required. Our company is growth focused and looking for a passionate individual who takes initiative, looks at challenges as opportunities and always follows through.

DUTIES AND RESPONSIBILITIES

  • Receive, responds and document client and referral inquiries regarding service provided
  • Conduct client assessment in their home, wherever home is
  • Ensure that the Service Agreement and other onboarding documents are properly completed
  • Schedule employees for care and service based on the client’s needs and the right employees
  • Develop the Care Plan and communicate it with the Care Team
  • Notify clients and employees regarding initial and ongoing schedules
  • Assist in achieving client satisfaction, which may include quality assurance calls to clients
  • Assist with recruiting, associate hiring, orientations, and in-services
  • Schedule and conduct caregiver orientation and set up training needs
  • Provide weekly reports on all your efforts and input notes regularly in the CRM system
  • Promote initiatives that generate revenue
  • Must work as a Care Provider if an HCA/PSW in case of staffing emergencies

Education/Experience Requirements:

  • Secondary School Diploma. Completion of a recognized Program, such as the HCA/PSW
  • Experience in home care, health care or senior-related industry required
  • Must possess a valid driver's license and reliable car
  • Proficiency with computers, learning new software and Microsoft Office (Outlook, Word, Excel)

Job Types: Part-time, Full-time

Salary: From $51,000.00 per year

  • Monday to Friday
  • Weekend availability
