The Pathways to Healthy Families program works in a holistic and culturally responsive manner to reduce harm to pregnant and/or parenting women who use substances and their families. Our program provides assessment, service coordination, case management, linkages with relevant social service systems, access to support and counselling and access to child development services including, individual parenting consultations, developmental screenings, parenting support, attachment strategies, support groups and workshops for parents as well as parenting resources and information.

How will you help to get us there?

As a Counsellor for Pathways you will:

· Develop the capacity for collaboration and provision of effective supports/interventions to pregnant and parenting women and their children in relationship to substance use and early childhood development through training, consultation and coordination with existing community and service networks.

· Work primarily from a local office located in Brampton, outreaching to services within the community.

· Liaise with existing pregnancy/parent outreach services to promote seamless and integrated services for women and their children.

· Build capacity for the provision of outreach to pregnant and parenting women with substance use concerns.

· Provide individual/group counseling, case management and referral to pregnant and parenting women with substance use concerns in collaboration with existing service providers.

This role reports to the Team Lead, Pathways.

What will you bring?

  • Bachelors Degree in Social Work, or a related discipline.
  • A combination of 3 years of work experience in the field of Mental Health/Substance Use.
  • Working knowledge of housing, health care and community resources.
  • Ability to use a range of technological tools, e.g. online reporting systems, videoconferencing etc.
  • Ability to work independently with minimal day-to-day supervision.
  • Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills.
  • Experience providing counselling and outreach services to individuals who are experiencing or have experienced problems related to substance use and mental health issues.
  • Experience working from a harm reduction perspective with individuals who are pregnant and/or parenting young children and using substances.
  • Experience working with the Child Welfare System in situations involving substance use.
  • Experience and/or interest in working with parents and children with special attention to the development of infants and children.
  • Access to a vehicle is mandatory.
