Partners In Community Nursing is looking for passionate, talented Registered Practical Nurses and Registered Nurses who specialize in Palliative Care to join our amazing team!

Our visiting nurses are part of a transformational change working in homes within Durham Region, by providing care that is Client-Centered in its approach along with caring for individuals who are in the last stages of their end-of-life journey.

Our team is excited to welcome you to become part of our diverse, collaborative care team, to provide services to our growing list of clients with in Durham Region.

The Visiting Program offers a variety of nursing services including:

  • Palliative assessments and end-of-life-care
  • Wound, Ostomy and Continence Care
  • Intravenous (IV) Therapy
  • Diabetic Management
  • Urethral Catheterization
  • Health Teaching and MORE!

Qualifications:

  • Palliative care experience is required.
  • Palliative care pain and symptom management experience is considered a strong asset.
  • Current registration in good standing as a Nurse with College of Nurses of Ontario (CNO).
  • Completed Palliative education, CAPCE, Fundamentals of Palliative care, LEAP or equivalent education in another jurisdiction is considered a strong asset.
  • Valid Ontario driver's license and reliable vehicle required
  • Must be available to work days, evenings, weekends, and some holidays.
  • Proficiency in English, both verbal and written required
  • Physical ability and resilience to provide care to clients
  • Experience working in the community setting is considered a strong asset
  • Experience as a Community Shift Nurse preferable
  • Excellent patient assessment and teaching skills
  • Must possess strong written and oral communication, critical thinking, clinical knowledge, time management, and customer focus skills
  • High degree of resourcefulness, flexibility, and adaptability
  • Strong morals and ethics, along with a commitment to staff privacy
  • New graduates are welcome
